Many of our customers are reimbursed for their customizable LOBAK with their Extended Health Benefits (EHB) and/or using their Health Spending Accounts (HSA).
Predetermination Requests:
Many plans provide either partial or complete coverage for customizable products such as LOBAK. Depending upon your plan details, it may be necessary to have a licensed healthcare practitioner prescribe and dispense your LOBAK.
To determine eligibility and receive your reimbursement, follow these steps:
Obtain a prescription for a customizable back support.

Reach out to your insurance plan provider for approval.
